Common Uses

When an official document contains inaccuracies, such as incorrect names, dates, addresses, or other key pieces of information, an affidavit of correction can be completed to fix the error. These affidavits can be applied to change information on:

  • Vehicle titles
  • Birth certificates
  • Death certificates
  • Marriage certificates
  • Real estate deeds

Vehicle Certificate of Title (By State)

Affidavits of correction are typically required when changing information on a vehicle certificate of title. The states below provide official forms through their Department of Motor Vehicles for this purpose.

In many cases, a new application for title will have to be submitted in addition to the affidavit/statement. State requirements should always be confirmed by visiting the appropriate DMV website or local office.
