Alabama Do Not Resuscitate (DNR) Form

The Alabama do not resuscitate form is a medical document that conveys a patient’s wish not to be resuscitated if they suffer cardiac or pulmonary arrest. In Alabama, this document is called a DNAR order (Do Not Attempt Resuscitation). The form is “portable,” meaning the patient’s wishes will be followed in any healthcare setting, even if they’re moved to another facility.
