A Tennessee petition for change of name is the court document through which an individual requests to legally change their name. It states that the petitioner is eligible for a name change and that they request a hearing to obtain an official court order.

- Must be a resident of the county where the petition is filed
- Cannot have a first or second-degree murder conviction
- Cannot be a convicted sex offender

How to Legally Change a Name in Tennessee
Step 1 – Gather Documents
The petitioner must complete the Petition for Change of Name and Order for Name Change of Adult, and have the Petition notarized.[1] In most jurisdictions, a county-specific petition will be expected. Petitioner’s should check the requirements in their county to ensure they have the correct form. The petitioner must also gather the following documents:
- Birth certificate
- Social security card
- Proof of residency
- Photo ID
- Passport (required in some counties)
- Marriage certificate/divorce decree (required in some counties)
Step 2 – File Petition
The petitioner must verify which court handles name change filings in their county of residence so they can
bring the above-listed documents for filing (either the chancery, circuit, or probate court).

Step 3 – Obtain Name Change
The petitioner must
attend their hearing so the judge can review their case. The court hearing may take place in person or online. If the judge approves the name change request, they will sign the
Order, and the petitioner’s name will be legally changed.

Name changes through marriage or divorce can be acquired without petitioning the court. A person may change their surname on a marriage certificate or return to their former name through a divorce decree. Official records can be updated by submitting either form to the applicable government office.
