Eligibility Requirements
- Must be 18 years old or older
- Must be a resident of the county where the petition is filed
- Cannot have a first or second-degree murder conviction
- Cannot be a convicted sex offender
- Minors must have the petition authorized by both parents (or guardians)
How to Legally Change a Name in Tennessee
Step 1 – Gather Documents
The petitioner must complete the Petition for Change of Name and Order for Name Change of Adult, and have the Petition notarized.[1] In most jurisdictions, a county-specific petition will be expected. Petitioner’s should check the requirements in their county to ensure they have the correct form. The petitioner must also gather the following documents:
- Birth certificate
- Social security card
- Proof of residency
- Photo ID
- Passport (required in some counties)
- Marriage certificate/divorce decree (required in some counties)
