The principal should sign the health care proxy in the presence of two witnesses and a notary public.
A mandate is a contract by which a person, the principal, confers authority on another person, the mandatary, to transact one or more affairs for the principal.[1]
The principal may revoke the power of attorney at any time, unless otherwise stated in the document.[2]
