Signing Requirements

At least two witnesses 18 or older must sign an advance directive.[1]

The witnesses cannot be beneficiaries of the principal’s estate or assets.

Advance Directive (Preview)

Revocation

An individual can revoke their advance directive at any time, in any state, using any method.[2] The revocation becomes effective once communicated to their health care provider. An agent’s authority can be revoked by written statement or by informing the healthcare provider of the revocation.[3]

Decrees of annulment, dissolution of marriage, and legal separation will also revoke a spouse’s authority as an agent.

Registering

Oklahoma law does not require advance directives to be registered. An optional registration system is provided on the State Department of Health website.[4]
