An Iowa living will form contains an individual’s wishes regarding life-prolonging procedures and is kept in their medical record for use if they become incapacitated. If the individual is in terminal condition or unconscious with no possibility of recovery, this document instructs their attending physician to refrain from prolonging their life and to allow a natural death.

An Iowa living will form contains an individual’s wishes regarding life-prolonging procedures and is kept in their medical record for use if they become incapacitated.   • Signing Requirements – Two witnesses or notary. One witness must be unrelated to the declarant, and neither may be their health care provider or employee.[1]

Statutory Definition

“Declaration” means a document executed in accordance with the requirements of section 144A.3.[2]
