Signing Requirements
A Minnesota health care directive must be signed by two witnesses or a notary public.[1]
The appointed health care agent cannot act as a witness, and at least one witness cannot be providing health care for the principal.

Revocation
The principal can revoke their health care directive by any of the following[2]:
- Destroying the document.
- Signing a written revocation.
- Verbally expressing their intent to revoke it in the presence of two witnesses.
- Creating and signing a subsequent health care directive.
