Signing Requirements
In Alaska, a medical power of attorney (designation of health care agent) has to be signed by two witnesses and acknowledged before a notary public.[1]
Witnesses cannot be:
- The healthcare provider or an employee thereof.
- The agent.
At least one witness has to be someone who is not related to the principal or entitled to their estate.

Revocation
The principal can revoke their selection for health care representative either in writing or by personally communicating it to their supervising health care provider.[2]
The advance directive portion of the form can be revoked in any way that clearly communicates that intent.[3] If there is mental illness detected, but the patient is still competent, revocation must be communicated to a health care provider.
In all cases, the individual must be mentally competent.
