Requirements

To tattoo a minor aged 16 to 17, the following requirements must be met[1]:
  • The parent or guardian must complete and sign the consent form and have it notarized on the same day as the tattoo.
  • During the tattoo, the parent or guardian must be present.
  • Both the parent or guardian and the minor child must present a photo ID.
