Adverse Reactions
It is essential to obtain information about the client’s medications and skin treatments, as some cause skin lifting or other adverse effects after waxing.
In particular, the client must have discontinued Accutane for at least one year before waxing.
What’s Included
Client Information – This section of the form is similar to a Waxing Intake Form. The client’s name, address, phone number, and email are collected to ensure accurate recordkeeping.
Patient Questionnaire – The client should be asked about certain information that may affect the waxing treatment and their reaction to it. This may include questions regarding:
- Medications
- Skin products
- Medical conditions such as diabetes, lupus, or cancer
- Menstruation
Side Effects – The consent form should include a brief disclaimer about possible side effects from waxing and a few examples, such as:
- Inflammation
- Discomfort
- Skin lifting
- Reddening
- Small breakouts
- Welts
Waiver of Liability – When the document is signed, this part of the form legally releases the waxing company from liability if the client suffers any injuries or side effects.
Consent to Treatment – When the client reads and agrees to the prior sections of the form, they sign the document to consent to the waxing procedures being performed.
