Citizens Bank Direct Deposit Authorization Form

A Citizens Bank direct deposit authorization form enables a company’s payroll department to issue paychecks directly into an employee’s bank account. The employee may designate up to three different deposit accounts and choose to divide their payments by specific percentages or dollar amounts.

Citizens Bank Direct Deposits: Authorization Requirements

Citizens Bank requires the employee to give their employer an authorization form that contains the following:

Name of company/employer issuing direct deposits
Name, address, SSN, and phone number of employee
Dollar amount or percentage of net pay to be deposited
Account number and type (e.g., checking)
Citizens Bank routing number
Signature and date

Finding Routing and Account Numbers – Routing and account numbers are displayed at the bottom left of checks. They’re also accessible through online banking or the mobile app.
