Citizens Bank Direct Deposits: Authorization Requirements
Citizens Bank requires the employee to give their employer an authorization form that contains the following:
- Name of company/employer issuing direct deposits
- Name, address, SSN, and phone number of employee
- Dollar amount or percentage of net pay to be deposited
- Account number and type (e.g., checking)
- Citizens Bank routing number
- Signature and date
Finding Routing and Account Numbers – Routing and account numbers are displayed at the bottom left of checks. They’re also accessible through online banking or the mobile app.
