Citizens Bank Direct Deposits: Authorization Requirements

Citizens Bank requires the following information to authorize direct deposits:

  • Name of company/employer issuing direct deposits
  • Name, address, SSN, and phone number of employee
  • Dollar amount or percentage of net pay to be deposited
  • Account number and type
  • Citizens Bank routing number
  • Signature and date
