Alaska Do Not Resuscitate (DNR) Form

An Alaska do not resuscitate form lets medical professionals know that a patient does not want to receive CPR should they stop breathing or have no pulse. This form, referred to as a Physician Orders for Life-Sustaining Treatment, can also be used to specify treatment preferences, additional medical orders, and medically-assisted nutrition options.

Signing Requirements

The form must be signed by a health care provider (physician, APRN, or PA).[1]

The signature of the patient is optional. If the patient is unable to make their own decisions, an individual designated by an advance directive, medical power of attorney, or certification of surrogate can sign instead.
