An Alaska do not resuscitate form lets medical professionals know that a patient does not want to receive CPR should they stop breathing or have no pulse. The patient can also specify treatment preferences, additional medical orders, and medically-assisted nutrition options. By signing this form, the patient is enrolling in Comfort One, and they should carry this form in their wallet, or wear a bracelet or necklace, so health care services can identify their status.

Signing Requirements

The form must be signed by a health care provider (physician, advance practice registered nurse, or physician assistant) after they have properly discussed the options with the patient or their representative.[1]

If the patient is unable to make their own decisions, an individual designated by an advance directive, medical power of attorney, or certification of surrogate would sign instead. The signature of the patient/representative is optional.
