Signing Requirements
A DNR must be signed by the patient or their authorized representative and the patient’s attending physician for it to be valid.[1]
The Colorado do not resuscitate form is completed by a physician and a patient who does not want medical professionals to attempt to revive them should their heart or breathing stop. Executing this form does not mean the patient won’t receive pain medications or comfort measures; it only applies to CPR and other resuscitation interventions.
