A Kansas do not resuscitate form instructs medical personnel that an individual has chosen not to receive CPR in the event of cardiac or respiratory arrest. This decision is often made by patients whose physician has determined it is medically appropriate due to a terminal illness or a state of health where resuscitation would be ineffective or harmful.

Signing Requirements

A DNR requires the signatures of the patient (or another individual at their direction), one witness, and the patient’s attending physician (or the PA).[1]

If the patient has chosen to receive treatment through prayer or other spiritual means from a member of their church or religion, then the physician’s signature is not required.
