A Kansas do not resuscitate form instructs medical personnel that an individual has chosen not to receive CPR in the event of cardiac or respiratory arrest. This medical directive is often implemented for patients whose physician has diagnosed them with a terminal illness and determined that resuscitation would be ineffective or harmful.

Signing Requirements

A DNR form requires the signatures of the patient (or another individual at their direction), one witness, and the patient’s attending physician (or the PA).[1]

If the patient has chosen to receive treatment through prayer or other spiritual means from a member of their church or religion, then the physician’s signature is not required.
