A Maryland do not resuscitate form is an order from a patient to health care providers not to perform CPR or provide life-sustaining treatment. This form (known as a Medical Orders for Life-Sustaining Treatment) is kept in a patient’s records to outline their desired course of action in emergencies, as well as preferences regarding antibiotics, ventilation, and other treatments.
