Signing Requirements

The patient and a physician, advanced practice registered nurse, or physician assistant must sign the DNR form.[1]

New Hampshire POLST Form – Like a P-DNR Order, this form gives medical staff do-not-resuscitate orders, as well as instructions regarding artificial nutrition and patient care.
