Eligibility Requirements
- Must be 18 years old or older
- Must be a resident of the county where the petition is filed
- Minors must have the petition authorized by both parents (or guardians)
- Cannot have a first or second-degree murder conviction
- Cannot be a convicted sex offender
How to Legally Change a Name in Tennessee
Step 1 – Gather Documents
The petitioner must complete the Petition for Change of Name and Order for Name Change of Adult, and have the Petition notarized.[1] Some courts may require that their county-specific form be used. The petitioner must also gather the following documents:
- Birth certificate
- Social security card
- Proof of residency
- Proof of ID
- Passport (required in some counties)
- Marriage certificate/divorce decree (required in some counties)
Step 3 – Obtain Name Change
Marriage and Divorce
Name changes for marriage or divorce do not require court involvement. A person may change their surname on a marriage certificate or return to their former one through a divorce decree. Official records can be updated by submitting either form to the applicable government office.
