The Georgia advance directive form is a document that individuals use to appoint a health care agent and record instructions for the administration of medical treatments in the event of their incapacity. A back-up health care agent should also be named in case the primary one is unavailable. Once the principal has been deemed incapacitated, the health care power of attorney will become effective and the agent will have the right to make health care decisions on their behalf. State law mandates that the agent must make all health care decisions in accordance with the principal’s instructions.
