The Iowa motor vehicle power of attorney (Form 411021) gives a vehicle owner the ability to appoint a representative (“attorney”) to act on their behalf regarding matters related to their vehicle. This includes the power to apply for a refund for a leased vehicle, receive a damage disclosure statement, receive a mileage statement, as well as application for and assignment of a Certificate of Title.
The type of authority that the attorney will have when representing the owner before the Iowa Department of Transportation can be indicated at the top of the form in the appropriate checkbox. The owner will be required to provide the vehicle’s details, including the make, style, year, vehicle identification number, model, and color of the vehicle.
