The Indiana tax power of attorney form (Form 49357) is a form that authorizes a representative to act on behalf of a taxpayer before the Indiana Department of Revenue. The form can assign a general authority that enables the representative to handle all tax types for all tax years or grant a limited authority to manage only certain years and tax types. This tax power of attorney provides spaces for up to four representatives to be appointed; however, there is no limit to the number of representatives the taxpayer may have.

After an Indiana tax power of attorney form is executed and made effective, communications from the Indiana Department of Revenue will be directed primarily to the representative(s). A representative’s powers will remain in place for five (5) years after signing the document if it is not otherwise revoked by a written and signed notice.
