The New Hampshire motor vehicle (Form Title 5) power of attorney is a legal form that allows a vehicle owner to authorize a representative (referred to as the “attorney” in the document) to register and title a vehicle on their behalf. The vehicle owner will need to provide the vehicle’s year, make, and identification number (VIN) in the form, which also requires the notarized signature of each owner. Once the power of attorney has been executed, the attorney-in-fact will be authorized to submit title and registration applications to the Department of Motor Vehicles (DMV) or local municipal office for the vehicle specified in the document.
