The Nevada motor vehicle power of attorney (Form VP136) is a legal form that names a representative (the “attorney-in-fact”) to carry out vehicle-related actions on behalf of a vehicle owner. The vehicle owner will need to provide the appointed representative’s name and address, as well as their vehicle’s year, make, model, and identification number.

Once the power of attorney has been signed by the vehicle owner, the attorney-in-fact will be authorized to execute documents in their name and apply for vehicle title and registration. If the vehicle’s certificate of title has been lost or is held by a lienholder, a secure power of attorney (Form VP221) must be used instead. This form will need to be purchased from a DMV office.
