The completed form will relay the general and specific powers the resident (the “principal”) grants their agent. This authority will not be affected by the principal’s incapacity or disability.

A durable power of attorney must be signed by the principal and notarized.[1]
“Durable power of attorney” means a writing or other record that complies with the requirements of Section 751.0021(a) or is described by Section 751.0021(b).[2]
