New York notary acknowledgment is used to confirm the legitimacy of an individual’s identity and signature on a document. A notary public will fill out the form to record the acknowledgment, which is simply a statement that the individual signing the document is who they say they are and that they acknowledged their signature on the instrument. If the signer is acknowledging outside of New York, the form must indicate the state, country, or territory where it took place.

Notaries in New York must renew their commission every four years (NY EXC § 130).
