Party Information

The names and addresses of the owner (releasor) and requesting party (release) must be provided.

Photo Description

The photos that are being released must be described on the form, including any identifying information such as content, context, titles, or filenames. 

Release

This section will state that the copyright holder/photographer releases all rights to the releasee to copy, edit, distribute, sell, or transfer the photographs. 

Consideration

Sometimes the releasor may require compensation such as a fee, royalties, or other exchange of value for the copyright of their photographs. The parties must agree to the amount before signing the form.

Signature

Once the document is complete, the original owner of the images must sign and date the form.

1. THE PARTIES. This Photo Copyright Release (“Release”) is made on [DATE] between:

Releasor: [RELEASOR’S NAME], with a mailing address of [RELEASOR’S ADDRESS] (“Releasor”), and:

Releasee: [RELEASEE’S NAME], with a mailing address of [RELEASEE’S ADDRESS] (“Releasee”).

2. RELEASE. The Releasor, being the owner and copyright holder of the photos, hereby grants to the Releasee the right of possession of the photos described below and grants permission to make unlimited printed or developed photographs for the Releasee’s personal or commercial use:  [DESCRIBE PHOTOS]

By signing this Release, Releasor affirms that they are the owner and copyright holder of the photos described above and agrees to the terms and conditions of this Release.
