An Alaska do not resuscitate form is a medical document that instructs healthcare providers not to resuscitate a person if their heart or breathing stops. The choice to avoid these measures (such as CPR) must be communicated by the patient, or their health care power of attorney if they are unable to do so. A copy of this form should be kept with the patient after it’s completed.

Signing Requirements

Alaska DNR forms must be signed by the patient (or their health care power of attorney), a physician or health care provider, and a witness OR a notary (not both).[1]

Formatting

As per state law, the DNR form must be printed with an orange background. The document may be printed on letter—or wallet-size paper.
