To be legally valid, the patient and their physician must sign the DNR form.[1]
In Florida, the DNR order must be printed on yellow paper.[2] The full DNR document or the wallet-sized order (the bottom portion of the form called the “DNRO Device”) must be kept where emergency personnel can quickly locate it.
