Florida Do Not Resuscitate (DNR) Form

A Florida do not resuscitate form directs medical staff to avoid CPR and other resuscitative measures from being performed on a patient if they stop breathing or their heart stops. The implications of the form must be discussed with the patient’s physician, who has typically diagnosed them with a terminal disease, end-stage illness, or the patient is in a vegetative state.
