Signing Requirements
The patient’s physician must sign this DNR form. The patient or their authorized representative must consent to the execution of the form.[1]
Out-of-Hospital
This directive is an “out-of-hospital” form in that it applies only to patients not currently admitted to a nursing home, hospice facility, or hospital. The form or an identifying bracelet should be kept on or nearby the patient so EMS can easily see it.
POLST Form
Patients who wish to indicate other medical interventions and artificial nutrition options in addition to a DNR can use the more comprehensive Physician’s Orders for Life-Sustaining Treatment form.
