The LaPOST form must be signed by the patient or their health care representative and the patient’s physician.[1]
It is recommended that a patient’s POST form be reviewed when there’s a change in their medical condition or they’re transferred to a different facility. The second page of the document can be used to record the different times and locations of these reviews.
