South Dakota Do Not Resuscitate (DNR) Form

South Dakota do not resuscitate form is used to inform medical personnel that a patient prefers a natural death and not receive CPR if their heart and breathing stop. The South Dakota Medical Orders for Scope of Treatment (MOST) form is a portable medical order that includes a DNR section and other end-of-life care preferences for patients with terminal illnesses.

Signing Requirements

The South Dakota MOST form must be signed by the medical provider and the patient.[1]

Comfort One

South Dakota also acknowledges Comfort One, a document and a special bracelet that indicates the patient does not want CPR, breathing tubes, shocks, or other life-sustaining measures from emergency personnel.[2]
