Rhode Island do not resuscitate form is filled out by a medical provider to state a person’s wish not to receive cardiopulmonary resuscitation if they have no pulse and are not breathing. The Medical Orders for Life‐Sustaining Treatment (MOLST) form also communicates the patient’s condition and additional medical preferences. The form can travel between care facilities.

Signing Requirements

Rhode Island MOLST forms must be signed by the patient (or representative) and a MOLST-qualified health care provider.[1]

Displaying the MOLST Form

Individuals should keep the MOLST form with them when they leave home. The form should be clearly displayed in their home. Copies of the document should be made and given to health care decision makers and family members.[2]
