New Jersey Durable Power of Attorney Form

A New Jersey durable power of attorney is used to give financial powers to a trusted representative. The individual (“principal”) can choose whether they want the document to go into effect immediately when they sign or only in the event of their incapacitation. The power of attorney will automatically be revoked upon the principal’s death.
Signing Requirements
The principal’s signature must be acknowledged by a notary public or other authorized officer.[1]
