Illinois General Power of Attorney Form

An Illinois general power of attorney is a legal document that enables a person to choose a third party to handle their finances. The principal (document preparer) can specify the parameters of the agent’s authority and when those powers come into effect. This non-durable power of attorney terminates upon the principal’s incapacitation.
Signing Requirements
Notary acknowledgment and the signature of at least one witness are required on the power of attorney. The witness may not be[1]:
- The attending physician or mental service provider.
- The agent or successor agent.
- Anyone who owns or runs a healthcare facility where the principal lives or receives treatment.
- A relative of the principal or a relative of any party mentioned above.
