Signing Requirements – The parent or guardian’s signature must be acknowledged before a notary public.[1]
Expiration – The powers granted to the agent are only valid for a maximum of six months.[2] This limitation does not apply to active duty military members, or National Guard and Federal Reserve members serving more than 180 continuous days.
