Signing Requirements

A Montana advance directive requires the signatures of two witnesses, and for a health care agent to be given powers of attorney, the document must also be notarized.[1]

Revocation

The principal may revoke their advance directive using any manner that communicates their intention to the attending physician, nurse, or other health care provider.[2]

Registering

Although registration isn’t required, an advance directive can be filed with the state registry by completing the Consumer Registration Agreement and mailing it to the address below.[3]
