Montana Advance Directive Form

A Montana advance directive outlines a patient’s preferences regarding life-sustaining treatments and selects a health care representative to make decisions on their behalf. The document only comes into effect when the individual cannot communicate or make health care decisions themselves.
Signing Requirements
A Montana advance directive requires the signatures of two witnesses, and for a health care agent to be given powers of attorney, the document must also be notarized.[1]

Revocation
The principal may revoke their advance directive using any manner that communicates their intention to the attending physician, nurse, or other health care provider.

Although registration isn’t required, an advance directive can be filed with the state registry by completing the Consumer Registration Agreement and mailing it to the address below.[3]
