CLEANING SERVICE CONTRACT

1. THE PARTIES. This Cleaning Service Agreement (“Agreement”) made on [MM/DD/YYYY] is by and between:

Service Provider: [SERVICE PROVIDER NAME], with a mailing address of [SERVICE PROVIDER ADDRESS] (“Service Provider”), and

Client: [CLIENT NAME], with a mailing address of [CLIENT ADDRESS] (“Client”).

2. SERVICES. The Service Provider agrees to provide cleaning services to the Client at [ADDRESS WHERE CLEANING IS TO BE PROVIDED]. The Service Provider’s duties include: [LIST CLEANING SERVICES] (“Services”).

3. SCHEDULE. The Services shall: (check one)
