How to Write
Step 1 – Amendment Date and Identifying Information
The effective amendment date should be added, along with the names and addresses of the employer and employee.
Step 2 – Contract Date
The date the original employment contract went into effect must also be provided.
Step 3 – Amendments
Most importantly, the agreed-upon amendments or additions will need to be listed, with references to the original clauses being altered.
Step 4 – Signatures
Both parties must sign the amendment. The signed document should then be attached to the employment contract.

AMENDMENT TO EMPLOYMENT CONTRACT
This Employment Contract Amendment (the “Amendment”) is made on [MM/DD/YYYY] by and between:

The Employer and Employee are referred to herein as the “Parties.”

The aforementioned employment contract is hereby amended as follows:
