An Arizona do not resuscitate form is a medical document that instructs healthcare providers not to resuscitate a person if their heart or breathing stops. The form establishes that the patient, or their health care representative if they are incapacitated, has decided they would like to die a natural death instead of receiving CPR. A copy of this form should be kept with the patient.

Signing Requirements

Alaska DNR forms must be signed by the patient (or their health care representative), a physician or health care provider, and a witness OR a notary.[1]

Formatting

As per state law, the DNR form must be printed with an orange background. The document may be printed on letter—or wallet-size paper.
