Signing Requirements
The DMOST form must be signed by the patient (or their representative) and a health care practitioner (physician, APRN, PA).[1]
A Delaware do not resuscitate form contains instructions for health care professionals on whether an individual wishes to receive CPR in the event of cardiac or respiratory arrest. A DNR order is included in the Delaware Medical Orders for Scope of Treatment, which allows patients to be even more specific with their treatment preferences.
