A Georgia do not resuscitate form identifies a patient who has chosen not to receive CPR in the event of respiratory or cardiac arrest. It is typically reserved for individuals whose doctor has determined that they are in an incapacitated state with no chance of recovery or have a medical condition that will result in imminent death or that would render CPR unsuccessful.

Signing Requirements

A DNR order must be signed by the patient’s physician. The patient or their authorized representative must consent to the execution of the form.[1]
