A North Carolina do not resuscitate form is used by an individual to decline CPR in the event of a medical emergency. This form does not prevent the individual from receiving other treatments, interventions, or care that they may need. As the form is “portable,” the order is valid in any medical facility as well as out-of-hospital settings.
