Signing Requirements

An Oklahoma DNR Consent Form must be signed by the patient (or their health care representative) and two witnesses.[1]

Certification of Physician

The second page of the document, the Certification of Physician, can be completed and signed by the patient’s physician if the patient is incapacitated and does not have a health care representative. In this case, the physician must have clear and convincing evidence that implementing a DNR would have been the patient’s preference.
