Signing Requirements
The South Carolina DNR must be signed by the patient (or their representative) and the physician.[1]
A South Carolina do not resuscitate form informs emergency medical personnel that an individual does not want CPR if they are in cardiopulmonary arrest. A licensed physician or osteopath must determine that the patient has a terminal condition before the patient can be issued a DNR order. The form must be clearly displayed and care givers must be informed of its location.
