Virginia do not resuscitate form is an order written by a physician that tells medical professionals not to perform CPR on the designated patient if their heart or breathing stops. The patient or their representative can revoke the DNR at any time by destroying the form or expressing their wishes to a health care professional.

Signing Requirements

The patient and the issuing physician must sign the durable do not resuscitate form.[1] The physician must have a “bona fide physician/patient relationship” to execute the order.[2]
