DELIVERY DRIVER CONTRACT

1. THE PARTIES. This Delivery Driver Contract (“Contract”) is made on [DATE] by and between [DRIVER NAME] (“Driver”), with a mailing address of [DRIVER ADDRESS], and [CLIENT NAME] (“Client”) with a mailing address of [CLIENT ADDRESS].

2. SERVICES. The Driver agrees to perform the following delivery services for the Client: [DESCRIPTION OF SERVICES] (“Services”).

3. TERM. This Contract shall commence on [DATE] and terminate: (check one)

☐ – On the date of [DATE].
☐ – Upon completion of the Services.

4. PAYMENT. The Client shall pay the Driver $[AMOUNT] per [PAYMENT BASIS (E.G., HOUR)] for the Services. Payment shall be made by [PAYMENT METHOD] every [PAYMENT FREQUENCY].

5. USE OF PERSONAL VEHICLE. If the Services are performed using the Driver’s personal vehicle, the Driver shall maintain a current vehicle registration and adequate automobile insurance, covering non-personal and commercial use as required by federal, state, and local laws.

6. USE OF CLIENT-ISSUED VEHICLE. If the Client is to provide a vehicle to the Drvier for use to perform the Services, the use of said vehicle shall be subject to the following standards and procedures: [RULES FOR USE OF CLIENT VEHICLE].

7. INDEPENDENT CONTRACTOR STATUS. The Driver, under the code of the Internal Revenue Service (IRS), is an independent contractor, and neither the Driver nor their employees or contract personnel are, or shall be deemed, the Client’s employees.

8. TERMINATION. Either party may terminate this Contract at any time upon written notice.
