An Illinois minor (child) power of attorney ensures that if a parent or legal guardian is indisposed or absent for an indefinite period of time, the parent can grant temporary legal authority to a trusted party to care for their child. The child’s parent(s) may use the form to grant the representative all parental authority, or they may state limitations in the appropriate field. The state of Illinois has no duration limitations on this type of power of attorney; however, an expiration date or event can be detailed. Once executed, this document renders all prior minor power of attorney forms invalid.

Signing Requirements (755 ILCS 45/3-3(b)) – One (1) Witness and Notary Public
