The Illinois motor vehicle power of attorney (Form RT-5) allows for someone (the “principal”) to give another individual (the “attorney-in-fact”) the ability to perform specific duties in relation to one (1) vehicle designated on the form. A vehicle identification number (VIN), as well as the make, year, and model of the vehicle, must be provided. A completed power of attorney grants the agent the power to do all acts as the principal might do if personally present. This includes the power to buy, sell, or transfer the title of the vehicle to another person.
