The Oregon motor vehicle power of attorney (Form 735-500) is the DMV power of attorney used to grant titling and registration powers to an attorney. This document can only be completed by the owner(s) of the vehicle and can only be for one (1) vehicle. The person appointed as the vehicle owner’s attorney does not have to be an attorney, they can be any individual or company who will be acting in a representative capacity. A description of the vehicle is required, including plate number, year, make, body style, and the VIN or title number. The vehicle owner must sign the form and, if applicable, the joint owner of the vehicle must sign as well.
