Oregon power of attorney forms are legal documents that permit an agent to represent a principal and make decisions regarding property, health care, finances, and minor children. Some powers of attorney are designed to remain valid while the principal is mentally incapacitated or disabled. These forms are categorized as “durable” and are wfell-suited for those preparing for end-of-life scenarios and medical emergencies. Alternatively, the principal may create a non-durable power of attorney that expires once they lose their capacity to make competent decisions.

Advance Directive – States a principal’s preferences regarding health care implemented when they become incapacitated. This package includes a durable medical power of attorney through which the principal will nominate their health care representative.

Signing requirements (§ 127.515(2)): Two (2) witnesses or notarization

 

Durable Power of Attorney – Transfers financial powers to an agent that remain in effect during the incapacity or physical disability of the principal.

General (non-durable) Power of Attorney – Provides an agent with financial powers that remain in effect until the principal loses their mental capacity or the document is revoked.

Limited Power of Attorney – Allows an agent to perform specific transactions for a principal. The agent’s powers are either limited in scope or duration of validity.

Motor Vehicle (Form 735-500) Power of Attorney – Appoints a representative to execute documents related to the titling and registration of a motor vehicle owned by the principal.

Minor (Child) Power of Attorney – Used by parents and legal guardians to assign parental rights to a temporary childcare provider.

Real Estate Power of Attorney – Authorizes an agent to represent the principal and conduct real estate transactions like buying, selling, leasing, and managing property.
