Required Information
It’s essential that the landlord and tenant specify the following information when filling out the release form:
- Original Lease – The form must include the start date of the original lease and the address of the tenant’s rental unit.
- Move-Out Date – Because a lease release terminates the rental agreement, the new deadline for the tenant to vacate should be stated.
- Payment – Landlords often receive payment from the tenant to end the lease early and free them from liability. Whatever amount the parties agree to must be specified in the release form.

LEASE RELEASE FORM
1. THE PARTIES. This Lease Release (“Release”) is made on [DATE] between:
Landlord: [LANDLORD’S NAME], with a mailing address of [ADDRESS] (“Landlord”), and
Tenant: [TENANT’S NAME] with a mailing address of [ADDRESS] (“Tenant”).
Landlord and Tenant are each referred to herein as a “Party” and, collectively, as the “Parties.
2. ORIGINAL LEASE. This Release represents the releasing of all obligations by each Party for the lease dated [ORIGINAL LEASE DATE] (“Original Lease”) in regard to the property located at [PROPERTY ADDRESS] (“Leased Premises”).
3. CONSIDERATION. This Release shall be signed by the Parties in consideration of $[CONSIDERATION AMOUNT] paid by the Tenant. Upon receipt, which is hereby acknowledged, the Landlord does hereby release the Tenant from all obligations and duties of the Tenant under the Original Lease and any other related commitments or liabilities. The Landlord acknowledges that their representatives, heirs, and assigns also release the Tenant from any and all claims, demands, and causes of action that the Landlord has or may have against the Tenant.
In consideration of the release set forth above, the Tenant hereby surrenders all rights in and to the Leased Premises. The Tenant hereby releases the Landlord in the same manner that the Landlord releases the Tenant, and after both Parties sign this Release, no Party shall have any obligations whatsoever to one another.
4. MOVE-OUT. The Tenant agrees to vacate and move out of the Leased Premises by [MOVE-OUT DATE] unless already moved out at the time of writing this Release. If the Tenant does not move out by the aforementioned date, the Tenant shall be in default of this Release and any Consideration shall be paid to the Landlord. Any and all claims, demands, liabilities, and causes of action shall remain in place. If for any reason the Landlord should need to contact the Tenant, the Tenant’s forwarding address shall be: [TENANT’S FORWARDING ADDRESS].
5. EXECUTION. In witness whereof, the Parties have agreed to this Release and have executed this instrument on the undersigned date.
